1) Our maximum age is 18, though we've got some plans in the works to keep demigods around after they turn 19 in-game. It's never really been addressed in-book what had happpened to demigods who weren't claimed at 13 before the promise was made and we've never had someone app in who isn't claimed yet, but we've installed a system of 'mass claiming' post-The Last Olympian so any of the demigods who weren't claimed past age 13 have now been claimed by their godly parent. If a player would like to, this can also happen in-game, like with the cases of Leo and Piper (who were past 13 at the time of TLO), especially if it's the first time a 13+ character has come to camp.
2) It is possible, but not for children of Pluto. In canon this really only happens to Aphrodite's children, where the demigod will be given Aphrodite's blessing and will be given a 'magic makeover' to look stunning.
3) Some weapons, such as Percy and Jason's swords and Thalia's shield, take the form of a household item when not in use. However, it seems that most of these items are small/handheld and unobtrusive (the pen, coin or bracelet) so the character could carry them on their person easily - so we're going to have to say no to the weapon taking the form of these large objects. It would be perfectly plausible for the weapon to take the form of a vacuum cleaner or broom for a mortal to see because of the Mist, though!
